COVID-19 Vaccination Submission

 

When directed by the Court to submit proof of vaccination, the individual so directed must submit proof of vaccination via the portal below.  Fully vaccinated means two weeks have passed since the second dose of a two-dose vaccine or one dose of a single-dose vaccine.  For additional COVID-19 protocol guidance, please review the court's General Orders regarding masking and vaccination.
